RTA Forms
The RTA produces the following approved forms to be used by tenants, lessors, and agents.  These forms can be downloaded from the list below. 
  Entry Condition Report (General Tenancies)
To be completed by lessor/agents and tenants at beginning of the tenancy.
 Do not send to the RTA.

Form 1a

  Refund of Rental Bond
To be used to begin process of claiming bond or part thereof. Form 4
  Entry Notice
Lessor/agents must use this form to serve notice of entry on tenants. Form 9
  Notice of Lessor's Intention to Sell Premises
Must be used to inform tenants of lessors' intention to sell the premises. Form 10
  Notice to Remedy Breach
Used to serve a Notice to Remedy Breach on a lessor/agent or tenant. Form 11
  Notice to Leave
Must be used to serve a Notice to Leave on a tenant. Form 12
  Abandonment Termination Notice
Given to tenant by lessor/agent if they believe premises abandoned. Form 15
  Renting in Queensland - Your rights and responsibilities as a tenant in general tenancies.
Must be given to tenants at start of tenancy. Form 17a
